Pain under right shoulder blade

I have been diagonise with mild lupus, for the last week I have had sharp intermittent pain directly under my right shoulder blade to edge of spine which seems to migrate into the side and front of my rib cage when I move, try to lie down on that side or laugh it’s almost like a spasm I suppose what I am wondering if this could be part of a lupus flare I am on methotrexate and plaquinel but cannot take anti inflammatory medication for inflammation as am on apixaban have been using heat rubs which I don’t appear to be helping to much

It is impossible to make a diagnosis without your doctor giving you a clinical examination. I would suggest, you make an appointment with your doctor and discuss your symptoms. It might be lupus related, or simply a pulled muscle! You can also ask about pain relief, which doesn't involve NSAIDs.

Heat is often helpful when it comes to muscle spasms.

It's also important you have a lupus specialist because not all rheumatologists are expert in lupus. Both Plaquenil and MTX are excellent medication. It will take some weeks before the MTX kicks in. Usually, you are started on a low dose, to see whether your body can tolerate it, before deciding on the dose you need.

You will also need to have support - from your partner, family, friends? Sometimes, it is better to talk to someone who is not connected to you because it is sometimes the case you can't speak about certain things, fearing you will upset someone.
