I subscribe to the MedicAlert charity, and I know they have links to insurers for such things.
If you go to the medic alert website, click on existing members there's a link to MedicAlert insurance services.

My travel insurance company had no problems continuing to insure me when I told them I had been diagnosed with ITP and this is worldwide cover including the USA. The only question they asked was whether I'd been hospitalised because of it. When I said "no" they confirmed that they would continue to cover me for no extra cost and that I would be insured if I had an ITP related episode whilst travelling.
So, my advise would be to check some of the regular insurers first before trying the specialist medical travel insurers, assuming that you haven't been hospitalised because of ITP.
