My Dad was admitted to the ICU in San Jose, Costa Rica on 1/21 and has remained ventilated since then, due to sepsis. Background is that he is 68 and is in heart failure - had pneumonia the week before that turned into sepsis. They stopped all sedation on Fri. 1/27 but he is still in and out of consciousness and not following any commands that the doctors insist they need to remove the tube. They are talking about replacing it with a trach but this is risky for him due to Hemophilia A and a lack of Factor 8 in the country. I don't know where to go from here. Even though he passes the breathing tests, they are afraid to remove the tube for risk of having to reintubate.
Do I insist that they try? I feel like he may improve without this tube that he hates so much (from past experience.) Do we wait longer on the ventilator? They're saying we're at a standstill where he isn't getting worse, but isn't getting better, and decisions need to be made.
I am heartbroken and stressed beyond belief. Any advice would be so appreciated 🙏🙏
