Hi, I have been looking for exact same as the Skipton fund ceased in November 2017. You have to go to the website nhs.bsa.nhs.uk cannot copy and paste The link. It is the EIBSS form so you can search that on the website. It is PDF file that you can download and complete. All the best, navs 😍 😍
