Have you been diagnosed with heart failure? Do you want to help other people with your condition?

We are looking for patient stories that tell your experience of heart failure. They can be written by yourself or you can make them anonymous. We want to provide support and help to those that need us, and your stories give us a voice and show people that they are not alone.
