Help please is it possible in a month for the angle to change from narrow to wide open angles? This just doesn’t make sense to me.

Hello again,

Well I am still in the system waiting for my next appointment on July 4th. My new consultant says I now have treated primary narrow angles, does that mean the laser has worked is this what opens the angles?

My letter on 17th April stated the following:

This was 6 Weeks after 2nd laser.

‘‘On examination today, she had visual acuity of 6/6 and 6/5 in the right and left eyes. Intraocular pressures, were 28 mmHg and 28.3 mmHg in the right and left eyes. Slit lamp examination shows bilateral patent PIs but narrow angles and shallow AC.

Is it possible in a month for the angle to change from narrow to wide open angles?

New Consultant

Letter 16th May

‘….. Both her anterior chambers show good peripheral anterior chamber depth with a large patent iridotomies. Gonioscopy shows wide open angles even though there are pigment clumping and angle especially in the left eye suggesting there might have an intermittent iridotrabecular touch previously. Overall at the present time, she does not have any angle closure.

I was then taken off the diamox which I was taking 4 times a day, as he said this was too much (always makes you feel better….), I was then asked to just take only pilocarpine 3 times a day and cosopt twice a day and have a follow up in a week.

On my next visit a week later 23rd May my pressures 21 and 20, and I was asked to take Pilocarpine 3 times a day, lantaprost at night.

A week later I was seeing rainbows around down lights in the morning in my left eye, I had a pressure check which was 30, so back on Cosopt for my left eye only twice a day,

I have been told to stop the Pilocarpine 1-2 weeks before by next appointment.

I am still scared about all of this, I wish I had never started any of it, as I am convinced I didn’t need it in the first place…. But now I feel I’m stuck in a loop…… I just wondered if anyone else had been in this situation and if they had any advice.

Hello, is there anyone that has any information regarding my post above?

Could it be the Pilocarpine eye drops that have created the open angle?

Or is my post just so bad no one wants to comment.

I’m very scared….

I can’t reply to your specific queries, but just to say it can take 6/8 weeks for your eye to settle after surgery and some time even to adjust to different eye drops. Sadly, as well the eye drops do seem to be a bit “trial and error”as everyone is different, so it can take a while to get the right ones sorted. The good thing is you are getting follow up appointments. Maybe make a list of questions to take to your next appt so you get all the answers you need. Good luck at the next appt.

The iridotomies you received open the drainage angle which is why the letter states you now have open angles. Regarding the concerns on your treatment and how it has gone, you would need to speak to your consultant. They will have the answers to your questions. You can do this by calling the eye secretary and they will rely them to your consultant.

Thank you Trish, I was just concerned as the 2nd Iridotomy was an enlargement in March and my subsequent visits stated narrow angles until I visited another consultant in May….

Could it take that long for the angles to open or could it be the pilocarpine drops keeping the angles open?
