NHS South West and SW Virtual Hub have partnered with Hope For The Community CIC to deliver online Hope Programme® courses to support people living with long-term conditions.

The Hope Programme® is a free 6-week (8 weeks for Long Covid) online self-management programme for people living with any long-term condition to help them meet others in similar situation, feel more in control and more resilient. Your members can refer themselves to this programme.

The course can be accessed from PC, laptop, tablet or smartphone. Participants will have access to the course 24/7. It takes about 2 hours to go through the materials each week, which they can do at their own pace at a time that suits them. It was co-created by people living with long-term conditions and researchers at Coventry University.

• Each week covers topics including:

• managing stress • smarter goal setting • gratitude

• character strengths • physical activity • eating well

• coping with fatigue • practicing mindfulness • communication and relationships

• The course includes worksheets, audio and video materials, quizzes, interactive activities and social networking via email and forums. Participants are supported throughout by trained facilitators with lived experience.

• Upon completion of the course, participants can continue to have access to the online community, course content, activities and peer-support.

Next course start date: 15th January 2025

To sign up or for more information, participants can go to h4c.org.uk/hpsw or scan the QR code on the attached posters.
