Strange flare ups?

I've always been a magnet for strange diseases but this has me puzzled. I began having extreme fatigue, weakness without losing muscle strength, extreme sleepiness, pain and high myoglobin levels. After seeing neurologists at a large research hospital for 10 years, DNA tests, muscle biopsies, I was finally diagnosed with a vacuolar myopathy similar to Miyoshi myopathy. Most of the time I'm just tired, no energy. But now I'm having episodes of extreme itching on both arms--no rash. It starts early in the evening gradually gets worse and then has stopped when I wake up in the morning. The only thing that relieves it is 'Icy Hot' roll on pain reliver that is 16% menthol. Sometimes I have times I'm in 'remission'--no itching. But then it starts again and I'm just wiped out. Doctor doesn't know what it is, blood work gives no indication. Does anyone else experience this? Could this just be a 'flare-up' of this muscle condition?

Tests show working fine.

Liver problems can cause itching. Have a liver blood test panel done.
