Something you might want to be aware of is that neuro-psychiatrists in the UK are at liberty to send different a different 'copy' of notes to patients than they do to primary care teams. In at least one instance the 'mental state report' was left out of a copy sent to a patient. The reason given for this was that patients wouldn't understand the language used which is more dumbing down than we need and/or indicative that they need to write more simply and explain any complicated terminology. Another reason is that these reports are of no therapeutic value to patients which raises an interesting question about why these assessments are done in the first place and why, if they have no therapeutic use, they are sent to your GPs.
