Help for 3 year old

Hello my daughter suffers from idiopathic constipation and was treated back in December with movicol upto the recommended 8 sachets and weaned back down as per protocol to a sachet on alternate days! For months she had minimal soiling and was just if she couldn’t get there fast enough.

However over the last few weeks she has started to soil her knickers again despite continuing with the alternate days movicol sachets.

We have a poo chart for rewarding her and a poo jar which she gets a small sweet from when she has 5 stars on her poo chart as an incentive. She sits on a toddler toilet seat with her feet on a step in the correct position and watches an iPad while doing her poos.

I have tried to see the GP regrading my concerns but see a different person each time and there’s no continuity, they tell me to be sensible and give 1-3 sachets daily or alternate days if I feel necessary.

I am now thinking that she may be impacted again and needs to be restarted on the initial disimpaction protocol, or that she is now having behavioural issues related to pooing.

Any help or advice would be greatly appreciated. I am aware that this is not an issue that is fixed quickly and takes time

Thanks

hi ,it does sound like she needs another disimpaction. Then when you go back down maybe try one sachet or even two per day . Senokot is very helpful with the sensation of needing to go. Pretty sure that it's not a behavioural thing. They tend to lose some sensation because the bowel stretches so they can't help it. If you haven't already watched it have a look at the poo nurses video, the link is on this page.

I’d try another disimpaction.

If that doesn’t work then perhaps she’s having too much movicol and that’s what's causing the soiling?

We’re currently going through a similar thing at the moment so trying to figure it out!

Thank you, I’ve started off the protocol today and I’m keeping her off nursery until she’s finished it and has been reviewed by the GP next week, she gets so embarrassed if she has an accident bless her.
