Prostaglandin E1 pain

I am almost 5 months past RARP. I have no erectile function. I am on a tadalafil and VED rehab routine from my surgeon.

Both my surgeon and urologist suggest I start injections. My urologist prescribed Prostaglandin E1. I have been injecting .2ml which gives me an erection suitable for use. I am getting an aching pain in my penis. At first, the pleasure overcame the pain but not anymore. Anyone else have a similar experience and can share what their medical professional suggested.

I had the same pain. Someone on one of these groups suggested that position afterwards had a large influence. He suggested that after I was done playing it was important for it to not dangle down or just lie flat against you when lying down. It worked for me. I wore underwear capable of keeping it in place and arranged it straight up. In the shower too, if I held it up the pain went away. I recall suggestions of warm vs. cold showers afterwards but don't recall which as it didn't work for me. Gentle massage, not for erection, but to help get the residual chemicals moving into the bloodstream should also be helpful. For me the "no-dangle" position did it; YMMV but experiment to find what works for you.

I also had the same pain. I spoke with my doctor, and he suggested gradually reducting the dosage by 0.02 ml each time (e.g. from 0.20 to 0.18 to 0.16, etc) to see if a reduced dosage would give a satisfying erection with less pain. I started at 0.16 and gradually reduced to 0.08.

Each time I had a solid erection, but slightly less pain each time. At 0.08, the pain is almost gone but the erection is still solid.

Of course, you should not experiment with dosage without consulting with your prescribing doctor. But it may be a conversation worth having.

I went down in dose from .2ml to .15ml last night. Erection not firm enough for penetration and much less pain. We will try .175ml later this week.

If you had nerve-sparing RALP, it can take up to a year to regain erections, assuming you had no ED before surgery. You may still get near-normal ones in a few months.

I had nerve-sparing. I did not have ED before surgery. I have been told anywhere from 6 months to 2 years to regain erectile function.

It takes a long time for the "spared" but traumatized (during surgery) delicate nerves to return to normal. In my situation, I had partial ED before surgery, complete afterward for over a year, then a slight re-awakening with the help of sildenafil (Viagra). The erections I did get were slightly different (I guess some nerves didn't heal) but still good. My orgasms were less strong, however, and I find that I masturbate much less than before surgery (I am single, unattached). My orgasms have improved some, but the libido just isn't as good. I hope your case will be different.

Have you tried trimix or Bimix? I use Bimix with minimal discomfort. Never tried PGE. 8 months after RP, it’s a waiting game...

I have varying degrees of pain, all tolerable, but sometimes there is a soreness which lasts for a number of hours. I think some of it is the amount injected and the location. Although I'm not 100% sure, closer to the base seems to be the most comfortable both for injecting and to minimize soreness

I have made several injections now and have settled on .2ml dose. It provides a useable erection in about 10 minutes. I have started taking two ibuprofen about 1.5 hours before my injection and this has reduced the pain to minimal. Still pain but tolerable.
