GFR drop 62 to 57 does that matter?

hi. I'm back with question. Reminder: i had declining GFR to 40s that began to go back up to 60 than 62. Latest it dropped to 57. Not very significant but it is back below 60. Should I be concerned or is it so close to 60 it doesn't matter?? is it likely to continue to drop? Or its wait and see i guess. How often should i be checked? My biopsy was clear, I have proteinuria which seemed to have gotten better but now more bubbles/ foam back in urine again.Any comments would be helpful. Glad to be a newbie part of this community. Hope people are doing well. Thank you.

Treat your kidneys right and you'll likely live a long normal life. Track your eGFR over time as a good indicator.

My mum's eGFR has also fluctuated around 60 but we're unconcerned (happy that it's ~2x higher than post nephrectomy ~6y ago). She also has some proteinuria but max 0.4g/24h so nephro unconcerned. Know your 24h fig?

I should've added that mums eGFR has nearly doubled due to a Nutritional Therapy protocol. Personally I prefer to trust a qualified NT for nutrition than unqualified medics.

Does your mum have diabetes or high blood pressure

Not now. She used to have high BP so took bisoporol but managed to wean off that after starting the NT protocol. Her BP is now consistently ~115/75 🤞.

Whilst never diagnosed diabetic, shes been at/close to pre-diabetes for many years (likely hereditary). That's taken longer to treat naturally but recent indications are that it is improving following further dietary tweaks of the NT protocol.

I am CKD 3b and since on prescribed diet my eGFR has improved. My Doctor has me tested every 6 months now.

I’m CKD 3a and have been left to get on with it by my GP. I asked to be referred to a Dietitian some months ago but have heard nothing as yet. Just wondering who prescribed your diet if you don’t mind sharing.

My Primary Doctor prescribed it. I am on Medicare and live in Florida, so hard to get referral to Nephrologist or Dietitian, especially since diet is working. My CKD diet is based also on bloodwork for potassium, phosphorus, and sodium, plus protein.

Thank you orangecity41. Good to know that your Primary Doctor is helping you and the diet is working, also that your eGFR has improved. But frustrating for you that you are having difficulty getting referred to a Nephrologist and a Dietitian. It’s natural that we want to do everything possible to improve our conditions.

All the nutritionalists seem to be for gastric intestinal can’t find a renal in my area. Any ideas re foods for living with one kidney to protect it for future.

Trends are most important with eGFR. Don't rely on a single reading - it can be effected by so many variables. The difference between 57 and 62 is negligible, and I wouldn't be too concerned. Stick with your diet and watch your medications (no ibuprofen), and see where the next tests are. You may just bounce up and down a little, but as long as it's not trending downward consistently, keep up what you're doing. No proteinuria is more important, and it seems like you've got that under control! Best wishes!

Thanks for response and best wishes. Ill relax about negligible drop. So I watch proteinuria: some bubbles are returning and I have urinalysis end of month. Best wishes to you.

You say you have proteiuria? How much? 1+, trace, ???? Normally on labs it's quantified how much proteinuria you have. If you do indeed have proteinuria I would recheck gfr in 90 days. Eat a ckd friendly diet and try to research why you could be having issue. I'm not sure your whole history and story, could be lots of things.

Hi.well I have mild proteinuria now. My protein creatine ratio dropped from1,399 to712 (200 is top normal) protein down to 46.5 from 174. My GFR had been 30s, 440s now 57. Don't know if these make sense to you. My protein in urine now dropped to trace. The foamy bubbles had nearly stoooed. NIW seems to be increasing again. Take good care.

Yes the trace makes sense, as that is a spot check. Is the protein down to 46.5 a 24 hour test measured in MG? The protein creatinine ratio I'm not familiar with. Normally if the foamy bubbles stop that is a good thing bc I had similar thing happen where I had lots of bubbles and then it slowly went away. Now and then I still have suds looking urine. You don't have any idea what is causing your issue? Everyone has a different story as I'm sure mine is different from yours

No, we don't know cause. Nephrologist says I'm bit of puzzle! Can't rule out that GFR decline and proteinuria all started right after my second bout with covid. Realize my text all messed up. It should have read that bubbles nearly stopped and now starting up again! It was not 24 HR test they like to do the protein/creatinine ratio test and feel 24 hour test less reliable as too many people do it improperly.Thank you for sharing, and responding. I see my nephrologist again in July so hopefully will know more then but my rheumatologist my check my GFR end of this month. Take good care.

CKD diet on the internet all seems to conflict with foods one encourages a food another says leave it out. Can’t find a site that seems to be consistent any ideas please

That's OK where your score is ,it's nearly normal in range just keep drinking plenty and keep on top of yourself and you will be good
