I am senior citizen aged 66 yeras ,diabetic detected in Jan 2008 ( 240/435) Well under control now.

My activities are regular Exercise 20 Min in morning ,walking 1 hour Or round of Golf. Strict diet Contol .

Now for last Two yeras Fasting /PP are 118/140 range .

Medicine 1 mg One tablet of AMRIL at night.

Is this Ok or I need change of Medicine /Routine

Do you mean "Amaryl"?

Glimepiride (generic name) Amaryl (brand name) is used along with diet and exercise, and sometimes with other medications, to treat type 2 diabetes (condition in which the body does not use insulin normally and, therefore, cannot control the amount of sugar in the blood). Glimepiride lowers blood sugar by causing the pancreas to produce insulin (a natural substance that is needed to break down sugar in the body) and helping the body use insulin efficiently. This medication will only help lower blood sugar in people whose bodies produce insulin naturally.

You should try to reduce fasting blood sugar less than 100 mg/dL and post-prandial blood sugar less than 140 mg/dL.

* Avoid single high concentrated meals.

* Small frequent meals, more times in a day.

* Food high in fibre content like vegetables, whole pulses, fruits, etc.

* Avoid consuming foods after 6:00 pm.(alternate medicine).

* Check your fasting blood sugar daily and avoid foods that cause elevated blood sugar levels. Post-prandial blood sugar may also be checked occasionally.

Ideal combination of diabetic diet should be:--

Carbohydrates: 55-60%.

Fat: 30%.

Proteins: 15-20% (0.8g /kg /body wt).

Fibres 40 g/day.

DIABETIC DIET

Do not eat at all:--

Sugar, jam, syrups, honey, sweets, chocolates, fruit juices, ice creams, cold drinks, glucose drinks, cakes and pastries,

sweet biscuits, puddings and protein powder with sugar.

Eat in moderation:--

Bread, chapatis, oat porridge, cornflakes (No sugar), Cheese, butter, Noodles or spaghetti, macroni, Thick soups, Milk &

milk products, Chicken,

lean meat, fish.

Take unrestricted:--

* Salads (cucumber, tomato, onion, raddish, salad leaves).

* Vegetables (cabbage, baigan, lady fingers, french beans, methi, palak).

* Fruits (except banana, chockoo or mangoes).

* Sprouted beans and pulses.

* Marie biscuit (with less sugar).

* Clear soups, Lemon juice, Buttermilk

Periodical tests to be conducted by diabetic patients:--

A: HbA1c. HbA1c is a lab test that shows the average level of blood sugar (glucose) over the previous 3 months. It shows

how well you are controlling your diabetes.

B: Control your high Blood pressure, if any. Normal blood pressure- <120/80 mmHg.

C: Control your blood Cholesterol: Normal cholesterol < 200 mg/dL. LDL <100 mg/dL HDL > 40 mg/dL Triglyceride < 150 mg/dL.

D: Diabetic education. Dental check-up should be done in every six months.

E: Eye examination for glaucoma, age-related macular degeneration, diabetic retinopathy and cataract.

F: Foot examination (Diabetic neuropathy and diabetic foot ulcers)

G: Glucose monitoring. Fasting or postprandial blood sugar may be checked daily and avoid foods that elevates blood sugar.

H: Health maintenance immunization against influenza and pneumococcal pneumonia.

I: Indications for specialty care. Heart, kidney and liver should be checked periodically by concerned specialists.

* Overall, the risk for death among people with diabetes is about twice that of people of similar age without diabetes.

* Adults with diabetes have heart disease death rates about 2 to 4 times higher than adults without diabetes.

* The risk for stroke is 2 to 4 times higher among people with diabetes.

Thanks you very much for the detailed answer,I will try to bring it down to below 100, In fact some months it is less than 100 but average fasting is 118 . I am following all the above precauitions,except However my specific Querry is should I continue with AMARYL 1mg once per day or are there better medicine ? This is since detecting Diabatic in 2008 I was on Dose of Twice dailly now reduced to One /day.

Dear Dr,

I agree with your comments on Allopathic medicine , however I tried Homeo medicne for couple of months but the FBS and PP increased so I had to revert back .

In all cases both Allpathy and Other drugs needs to be taken simultaneousely for some months ,and during this reduction of Allopathy ,Bood sugar level increases.

In case you have some proven alternatives please suggest
