Conducting a survey

Hello everyone..

I'm new to this community. Well, I am from a pharmacy college and am working under a research regarding diabetic patient medications and to derive a statistic according to patient demographic. I would be highly obliged if people would come forward and help this cause. We have created a survey form, which I intend to post here till tomorrow.
