Hi, I struggle with constipation, bloating and pain, and a constant feeling of the need to go to the toilet. This feeling is almost constant and creates additional problems with my sleeping, which already is pretty bad. Anybody else with similar problems, any suggestions? I use various laxatives, without much success. It seems either the dose is too small or too large. I would not say this makes me crazy, but not very far from. It does definitely constrain most social life I might have.

Have you tried Thiamine ?
High-dose thiamine relieved my Parkinson's related constipation within the first two weeks. It restored normal peristalsis. Sometimes I do have excess peristalsis which does make me feel like I need a bathroom visit only to find there is nothing to expel. This can be mitigated by adjusting dosage.
In my case I also suffer from insufficient moisture in my G.I. tract. The stool softener known as sodium docusate fixes that problem for me. It restores normal water content.
A lot of PWP have a slow, and most often incomplete, empty of their bladder and/or stomach and/or bowel. If you feel like you have to go, may likely be because you're not empty, i.e., your backed up, you do have to go, so you have to employ a regimen of anti-constipation measures which you probably heard by now. Exercise, a lot of water, fiber, psyllium fiber husk, stool softener, Taco Bell, etc.
What kind of laxative have you been using?
You might find the use of Mag-07 (Aerolife) to be helpful. Go to Amazon and read the testimonies to its effectiveness.

According to forum members who have started High Dose Thiamine (HDT), one of the most commonly reported benefits, of the many that they report (80 reported benefits) is an end to years of constipation!

Others on the forum have reported that certain magnesium forms such as magnesium citrate or magnesium oxide are also useful for relieving constipation. Some people have reported that even topical magnesium chloride oil spray (mag oil) that they were applying for pain and cramps associated with PD, also helped ease the constipation.

My husband who also has PD found his solution to constipation by eating honey coated peanuts!! I know it’s not a medical solution but it certainly works for him. He eats a large handful each day and also takes two senecot daily. It’s made a huge difference for him!
Taking high dose Thiamine seems to be an extreme treatment for constipation.
I recently started taking a probiotic, a glass of Kevita everyday, and it has been very effective at relieving my previously chronic constipation. You may want to try that first for a couple weeks.
I don't think that urinary urgency is a PD thing. It usually relates to an enlarged prostate which is normal with age and can be treated with meds but could be more serious. You should see an urologist ASAP.

I make smoothies daily with chia seeds and whole fruits like apples with the skin. Good way to take supplements, just open the pills and add to the contents.
A good probiotic before bed. Prunes if necessary. Magnesium glycinate for tight muscles and mag threonate for the brain, the magnesium helps constipation too.
