Hello Claybuster, not sure what nucleated RBCs mean but if that bone marrow is stressed then your lab numbers will most likely start treading down. Did I read that your Spleen is enlarged? I did read that your docs recommended O&V, maybe since that treatment can get in there over a short period time & get you to remission quickly. The BTKIs have bleeding issues as a possible side effect & if you’re already on blood thinner, I guess they want to minimize any major issues as best they can. Starting treatment takes into account many things! #GODSPEED🙏🏾

Keep an eye on your haemoglobin and platelet trends, which are among the primary considerations for starting treatment. Basically, per the iwCLL, platelets dropping below 100 (lower than 100 is acceptable provided counts are stable) or haemoglobin dropping below 10 are the triggers. Your ALC doubling in under 6 months is a sign that closer monitoring is needed to determine whether other symptoms show that CLL is becoming active enough to warrant starting treatment.
