CLL can't spread - it's already everywhere in your body where you have a blood supply.
It can sometimes transform into an aggressive lymphoma (Richter's Transformation/Syndrome). The risk is a few percent depending on markers.

Also - blood cancers increase the risk of secondary (other) cancers.

People with CLL can get any type of second cancer, but they have an increased risk of:
Skin cancer
Melanoma of the skin
Cancer of the larynx
Lung cancer
Colon cancer
Kaposi sarcoma
Soft tissue sarcoma
I recently had a second opinion video conference with a CLL Hematology Specialist at the University Health Network in Toronto Canada. She told me that I had a 2x greater likelihood of having another form of cancer and a 10x greater chance of getting skin cancer. Needless to say I'm wearing sun hoodies, big brim hats and using sunscreen every day.
