My brother has been taking ibrutinib for more than a year. As his abdominal lymph nodes began to enlarge, doctors have switched Ibrutinib to Venetoclax 10mg for the past 3 months. His WBC has improved but hand R:B:C, Hemoglybin and Platelets have decreased greatly so he had blood and platelets 2 times. Has anyone had such an experience after using Venetoclax?

I had Venetoclax 300 mg for one year, until April 2022. Planned for another year but had to stop because of covid infection. Was Umrd in March 22, no cancer cells detectable in blood.
I have a complication ITP since many years with low platelets. Venetoclax is said to not impact the platelets, but who knows! I take other medicines for low platelets. It it goes down very low I get IVIG ( immunoglobulins) infusions, keeps them up for about two weeks.
I never had blood or platelet transfusions. For me with ITP transfusions would only last a day or couple of hours dr said.
The main problem I had with Venetoclax was stomach occasionally total blow outs......

I was on Venetoclax 400mg /day for 6 years and just stopped last week due to low RBC, Hct, HGB. My doctor suspects I have early AIHA ( Autoimmune Hemolytic Anemia ) ncbi.nlm.nih.gov/pmc/articl...
which can occur in CLL patients in or out of treatment, but usually more common with treatments like Venetoclax / Venclexta or Obinutuzumab / Gazyva
lists the side effects reported for Venetoclax and the table shows that 17% to 20% of patients have anemia, with 7% to 8% having grade 3 or worse, which may be the level your brother is experiencing.

And your brother may have started Venetoclax at 10 mg but under normal treatment guidelines he should have had weekly increments over 5 weeks to the usual 400mg level- see venclextahcp.com/cll/dosing...
