Confused fibrosis or Cirrhosis

I have been suffering type2 diabetes since 2007. I went to my doctor in Sept 2019 after detecting an increased SGPT 74 U/L during a routine check. As per doctor’s advice I conducted USG and diagnosed Hepatic parenchyma is slightly inhomogeneous in echotexture. I took Ursodeoxycholic (150 mg) Tab for 2 months (2 Tab par day) during Sept 20, 2019 to Nov 20, 2019 as suggested my doctor. Then SGPT level came down to 30 U/L.

I have undergone a second and third USG on 5th and 8th January 2021 and diagnosed i) Hepatic parenchyma is inhomogeneous in echotexture ii) Parenchyma is course in echotexture respectively.

Now I am taking silymarin 140mg Cap (1 per day) as per my doctor’s advice.

My doctor did not tell me clearly if it is fibrosis or already developed cirrhosis. Please advise me what can I do. Is there any hope for me?

Coarse in echo texture and inhomogeneous in echo texture both mean the same i.e. the liver isn't normal, it isn't smooth as it should be and this can occur due to fatty change, fibrosis and cirrhosis. With diabetes type 2 it is probable that you have some degree of fatty liver disease - this is reversible with lifestyle changes. If it has progressed as far as fibrosis this is still reversible and even early cirrhosis is too so it isn't all doom and gloom.

Doctors in the UK wouldn't advocate silymarin but if your doctor advises it then he obviously thinks its ok in your situation.

You might find the advise about Non Alcohol Related Fatty Liver Disease (NAFLD) of some use to see if there are any lifestyle changes you can make that can help reverse your liver issue.

Hi AyrshireK, thank you so much for explaining the liver conditions appearing in my USG report. I have forgotten to state my symptoms that I have been experiencing loss of appetite and also right side abdominal pain for last 15 days, which is continuing till now. Last week my S ALT was 34 and SGOT 19 and two months ago (early Nov 2020) S ALT (SGPT) was 29, S.Creatinine was 1.3. I am still worried with the symptoms stated above. Can you please advise me?

I can't really advise on particular symptoms - what you describe can be common with liver disease of many types. If you follow the advise for NAFLD then your liver may well improve and this in turn should remedy symptoms.

If symptoms persist see your doctor again.

This should be something your doctor should discuss with you, they are the ones who have the full picture as regards all your test results in front of them - we sadly will all only be guessing.
