I have been having fluctuating alt for 6 years. Been through all the blood tests and scans and have asma antibodies and high igg. I have had autoimmune thyroid disease and have lupus. Things point towards AIH however my alt has recently gone down to almost normal so hepatologist says he won't do a biopsy unless alt goes back up which is understandable. However I was hoping to have a biopsy to get some definitive answers. My question is does ALT often fluctuate with AIH? My hepatologist said normally it gets progressively worse rather than going up and down. A case of watch and wait.

I hqve PBC/AIH overlap. Was diagnosed with PBC first and then the AIH later. When my alt kept rising it only came down with tye steroids and the azathioprine. I don't know if that is the norm or whither ot varies with different people. Sorry i could not help. Ayrshirek is very knowledgeable in AIH and im sure she will be on at somepoint and can advise .
