Dear all,

We would really appreciate it if you could give us any information at all from a patient/carers perspective in relation to chronic hepatitis C. The patient/carer perspective can really make a difference and can ultimately have an impact on which medicines are routinely available for use in NHS Wales.

Please answer any sections of the questionnaire that you can. Do not worry if you cannot provide information on the medicine itself (daclatasvir). We are interested to know about how the condition affects patients in their day to day life and how their current treatments (and previous treatments) affect them.
