Hi been on Ramipril 5mg for 8 years or so and six months ago noticed blood pressure was in the 150/70ish mark. Contacted cardiologist for advice and he just said up your dose to 10mg once daily. Blood pressure in the morning first thing is still around 140/70ish but drops during the day to 112-120 ish. Asked should I take 5mg twice daily to try and curb this morning spike and was tpld its better as a once a day treatment. There are various combo blood pressure meds that seem to work better apparently but I dont like to deviate from instructions as i have a left hand stent and EF of 53% which the ramipril is helping. Also do I need further blood work foe hyperkalemia due to the increased dose.

Dear wischo,

You will need blood tests done with the increase in Ramipril, I was told that they can disrupt the function of the kidneys.

Before I had the bypass I was on 10mg a day, 5 in the morning and 5 at night. Once I had the go ahead for the operation, I was taken off the Ramipril.

Unfortunately my operation was cancelled 6 times over a 12week period so I was off the Ramipril for that time.

When I was restarted it was at 1.25 mg morning and the same evening, blood tests then upped to 1.25mg morning and 2.5mg evening, blood tests then upped to 2.5 morning and 2.5 evening, blood tests then upped to 2.5 mornings and 5mg evening, blood tests now back to 5mg morning and 5 mg evening.

This was over a period of 15 or so weeks

I have been very lucky and have had no side effects maybe because it was done this way?

Hi Wisco I had blood pressure that was doing the same. Had the morning spikes. I was on Slozem and ramipril. I had to come off ramipril due to the cough it gave me but am now on candesartan and No longer have the morning spikes. For me it has been a wonder drug and am giving regular 117/ 73 readings now. I hope this helps sharing this with you

Go back to cardiologist or ask GP for refferal.... Why ask advice of a specialist as in your cardiologist then ask this forum!!!!!Professional advice for your condition is key.. Not getting the answer you want can lead to too many cooks!!

The same could be said for any other question on this Forum. As with Covid running rampant it is difficult to get an appointment and some experienced people may shed some light on the question. If your theory is correct then this forum is pointless anyway, everybody here asks for advice from their GP or cardiologist and clearly they need some more ideas from people with similar situations.
