Isosorbide mononitrate

Hi ,I was put on 30 mg of isosorbide mononitrate by the hospital last Tuesday. They certainly help with the sharp pains during the day but by the time evening comes the pain comes back.. has anyone else had this ??

I was on it beforey bypass. My understanding is that it can be taken twice daily. Why not discuss with your GP doing this. You would probably need 20 - 25mg morning and evening.

Thanks Michael, I will definitely give my doctors a call tomorrow morning. It is really messing my sleep up. I’m dreading going bed as I know I won’t get to sleep.

As it has a half-life of 5.1 hours taking it twice (or even thrice) daily actually makes sense. Two of my worse angina attacks were between 07:00 and 08:00pm when I took them once daily.

I normally take mine between 8am and 9am I’m all good up until 3pm . So what you say is right. I took my GTN just now it’s eased but not completely gone 😬😬

Ahhh that explains why my prescription says take one am and other 2pm. I found it strange that my other tablets are either am or pm and this was was specific. Thanks...

Hi I am on 40mg twice daily but take one am and the other at 2pm

Great timing on your post Lynn. Had a stent in LAD in february this year but I don't like to bother my GP. I was considering upping IM (40mg morning) to twice daily also. Discomfort , poor sleep,exhausted all the time and strangely anxious. Let me know how it goes with GP.

Hi, Iv been doctors this morning been told to take another dose around one o’clock to see if this helps. Fingers crossed. All the symptoms you have mentioned is the same as mine😬😬 please check with your doctor before upping your medication.

There is a longer acting version I am now on. You take just one a day.

Hi, I was told to take one pill after breakfast and a second pill 6 hours later. That works well for me for the last 12 months. Good luck. Clare

Hi Lynn, yes in the beginning they have to take a bit of a starting point for dosage. You may need a higher dose so it is worth speaking to whoever prescribed them for you. In general having the pains at night is much worse than during.the day

Hi mk1954 Iv been to see my doctor this morning. They have told me to take another 30 mg around 1pm . They’re not totally sure if this will help as they are not convinced it’s heart related or if it’s reflux.. my BP was low and want me to go back tomorrow my to get it checked again.

Hi, are you taking them 8am 2pm, it might be you need your tablets changing. Mine keep changing all the time .

Yes I will be healthyheart1. I was only on 30mg in the morning now I have to take another one at 2pm.

I have been on 30 mg once a day and I take it before goingg to bed. I have been on this dosage for 2 years and6 months after I had a pacemaker, and have had no pain.

Hi Lynn. I realise this is an old post but I just wondered what conclusions you came to regarding your health. My partner is currently confined to bed and after numerous bloods and scans no diagnosis has been made. She has pain in the left side of her chest,shoulder & back and so they started her on isosorbide mononitrate XL and like you she feels the pain worsens during the 8-10 hour period after taking. However, like you they are querying gastro problems as she has discomfort and is struggling to eat. All ECGs, echo and CT scan of heart have showed normal but she's awaiting the CT angiogram to check her arteries. I suppose for us the question is much like yours was. Does she need a higher dose or is the tablet the right thing to be taking?
