Here is the link to guidance on NHS Wales for shielding, which you say your doctor said:
"Guidance on shielding and protecting people defined on medical grounds as extremely vulnerable from coronavirus (COVID-19)" published on 24 March

In particular it says this if you start to feel unwell - with a link under 'online coronavirus service'.
Provide information on who they should call if they feel unwell, how to use the online coronavirus service, and leave the 111 number prominently displayed.
Hope this helps. We currently have no more specific information for NHS Wales.

Apologies. It also says:
You will receive a letter from the Welsh Government setting out the advice and sources of help and support in your local community. If you are employed, this is also proof that you will not be able to go to work while you are shielding and can be shared with your employer. If you are able to, you can work from home, if your job allows it. You will not need to get a fit note from your GP.
So you need to get back to your GP about this. I am sure he will need to do something to trigger this response.

I am in NE Scotland and in a similar position with no letter so yesterday when the phone rang and a administrator from my GP practice advised me my monthly blood tests would be now scheduled for 3 monthly due to coronavirus I took the opportunity to ask if I would be receiving a letter?
She wrote all my questions down and advised she would ask a GP, within 20 minutes a GP called and advised me I should be sheilding because I was taking Azathioprine and I should receive by the end of the week a letter from the NHS/government, I take that to be the Scottish government, she advised me everyone's details are being looked at and contacted if they fall within this group but things are running behind and I should start sheilding straight away and not wait for a letter, I said I had been isolating for the last 2 weeks, I live very rural and have not spoken to anyone other than my husband who has been isolating too.
GP said I now need to be sheilding and if my husband was going out anywhere then we should be sleeping separately and not living closely together, I said the only reason he would need to go out was to collect my prescription, she advised completely against this and then gave me a couple of people's names and a Facebook link for my nearest village, 5 miles away, has set up a group of people to deliver prescriptions and food etc.
I discussed the shortages of my medication from the pharmacists over the last 4 months and she said she was not aware of this but would investigate this, but then when I asked to increase my prescription from the one months supply I am allowed to have in this NHS region, she said that the Scottish government had advised not to do this..
They only prescribe 2 months prescriptions of any medication in Scotland but because people do not turn up for routine blood tests for Azathioprine this has been put on monthly supply only...
