Aldara works by stimulating your immune system in the skin areas that you apply it to. You can often feel this as a warm heat sensation (like when you've caught the sun sun) which means it's working. However, if the area becomes too sore or the skin breaks you probably need to have a break from using the cream until it settles. If the warts are still there after that start using the cream again with either less cream or using it fewer times in the week.
As for how long it takes to work, in your case it sounds like it's working but it's variable how long it takes.
If you are having problems I'd recommend you head back to your local sexual health clinic and have a chat with them / let them have a look at the anal area.
As Aldara works with the immune system it takes time to be efffective. Some people notice the warts shrinking after 2 weeks, but in other cases they may not go away for several weeks, or even months.
It is quite normal to experience itching, swelling and burning at the site of the wart. It actual means the cream is working, rousing your immune cells to fight the virus.
If it becomes too uncomfortable stop the treatment for a few days, until the skin is less sore.
Contact your Sexual Health Clinic if things do not improve for further advice.
