how effective is doxycycline to treat chlamydia? has anyone had the STI come back after treatment?

It's very effective provided the course is taken properly. There aren't any doxycycline resistant chlamydia bugs.

It is possible to get a false positive result for up to 6 weeks after treatment, so if this is the case then you shouldn't be worried just get tested again after the 6 weeks is up. There was also an MSV strain of Chlamydia going around last year which is more resistant to antibiotics and required 3 weeks of treatment instead of one but still not anything you should be seriously worried about.
