Are your family aware of your struggles and if so can they ask school about a meeting to discuss how best to support you. Are you waiting for an assessment? If so then the school might be able to help as they normally write a plan to support you in school with or without an assessment. You can also ask the Dr to write to the school on your behalf to state your difficulties. .
Agree with Bee - bop, also for Diagnosis, ask Gp to contact CAMHS & write to CAAS they are the Learning Disabilities, Difficulties go to for Forms, Diagnosis etc
School should contact SEND for you to receive Support & assess your Support Needs.
