I have bronchiectasis and understand the importance of getting the right antibiotic for any chest infection. Whilst I have a few pots to use, I don't have any forms to go with them. Is this normal practice? It delays my getting a sample sent off because I have to see the doctor first and then provide a sample that coincides with the next collection time. I find it hard to get the stuff up on demand! Thanks for your thoughts on this.

Phone your consultants secretary and ask her to contact your consultant team so one of them can arrange for a form. When you next see your consultant in clinic ask for forms in advance. All the best. Marlene
I hand my sputum samples in at the GP surgery when I feel I’m getting an infection. When I need to restock on pots they give me the little bag as well to seal it in with the form attached which i fill in before handing in for collection. I do have an arrangement with the surgery to do this so maybe it’s something you can ask about next time to save having to see the doctor first.
Hi i have same condition they should give you the l form and bag at the same tine they give you the bottle or pot so u get sample when u can and date it and sebt it i would ask for the form next time u see ur doctors or coulsant at the hospital

You should be able to hand the specimen to the receptionist or nurse to bag it up. That’s what I have always done. No need to see the doctor.
I have trouble with this as the hospital I attend isn’t local and is paperless. I usually take in a copy of the numerous letters I have from my consultants saying I have to give in sputum samples and they generally accept that eventually. I have had samples rejected because they were sent without paperwork and because it was in the wrong shape pot (supplied by my hospital)! I probably wouldn’t be able to get an appointment with a gp anyway so this is the least painful route. Seems every surgery is different.
