Entresto and COPD.

Hi All, I have been selected to try Entresto for my heart failure side of things and have read very good reports about it from cardiac patients. I am interested to know if anyone else who suffers from both HF and COPD has been prescribed this and whether they have benefitted from it.

I have asked the same question in the Heart Foundation part of H U without reply so thought I would try here,

Hi John

My husband has had right sided heart failure for over a year now and he also had a cardiac ablation procedure for a heart flutter, which doesn't sound serious but he had a heart rate of 170 for 3 days ( normal is 60-100) and it did not respond to "a shed load of medication" ( cardiologist's own words ). Since the procedure his heart rate has been fine but he has to take blood thinners and furosemide ( water tablets) which he's not keen on but seem to be the lesser of 2 evils. I've not heard of Entresto but having looked it up it's specifically intended to treat heart failure and has better results than previous medications.

It's expensive $12.50 per day so maybe that's why we've never heard about it. It's been around since 2015.

Good luck with it and let us know how you get on. I'd be most interested in finding out.

Hi Corin, thanks for your reply and I will certainly post here again if all goes well during the side effects trial and tablet strength build up which, in the UK, has to take place over a 3 month period in the hospital outpatient clinic. The cost to our NHS in the UK is £45.78 for 28 tablets. Below, I have posted the NHS criteria for this heart failure medication.

NICE guidelines for Entresto (Sacubitril Valsartan)

Sacubitril valsartan is recommended as an option for treating symptomatic chronic heart failure with reduced ejection fraction, only in people

with New York Heart Association (NYHA) class II to IV symptoms and

with a left ventricular ejection fraction of 35% or less and

who are already taking a stable dose of angiotensin‑converting enzyme (ACE) inhibitors or angiotensin II receptor‑blockers (ARBs).

1.2 Treatment with sacubitril valsartan should be started by a heart failure specialist with access to a multidisciplinary heart failure team. Dose titration and monitoring should be performed by the most appropriate team member as defined in NICE's guideline on chronic heart failure in adults: diagnosis and management.

Hi I have copd and heart failure due to chemo therapy. I have been on entresto for nearly 12 months and improved so much along with frusemide and betablockers. It improved my breathing as well. Maybe its worth a try.

Hi there, Thanks for responding, I am so pleased for you that your HF has improved.

I have LH HF. LV systolic dysfunction with ejection fraction 33% and am currently on Ramipril, Bisoprolol and 80mg Furosemide. I hope that the Entresto will also suit me when I start my treatment on the 30th of this month, fingers crossed. Once again, thanks for the response.

Hi again kpm, I certainly will let you know how I get on. I have been waiting for almost 12 months for this treatment so I am hopeful that it will suit me. Hope you're not melting down under.

Forgot to mention that I am also on Oxygen Therapy 24/7 so hope that my breathing will improve too.

Hi John it is autumn here and very pleasant but winter is in the air. Forgot to mention my ejection pre entresto was 37% now it is 45%. Also there is a trial on the go which I have joint at request of my cardio in conjunction with entresto to strengthen the heart muscle but it will be awhile yet before they collect all the data. My hubby has advanced HF and fibrosis of the lung (which they thought for years was COPD). He has been on Entresto for 2 years and I think it has helped the heart side of things as it seems to have slowed the progression a bit....sadly though he is deteriorating bit by bit every month now. But yes, it helped for ages and we are thankful for that. Go for it! You have nothing to lose!

Thank you for taking the trouble to reply even though your hubby is so poorly and I am so sorry that he was misdiagnosed for so long. It took a visit from the Community Matron to me to get things moving, including a very firm phone call, including putting a receptionist in her place, to my GP asking why no-one had sent me for tests for heart failure as my symptoms were obvious to her. It does often seem that patients with COPD and other respiratory problems get treated in a fairly casual manner by some medical practices so that it sometimes takes a good kick up their backside from someone who is competent to recognise their inadequate handling of it. Fortunately for me, the Community Matron is an ex ER Sister who has hands on experience of both COPD and HF, and to say that she was quietly seething is an understatement. She also eventually spoke to the highly qualified cardiac nurse at the hospital where she worked and who now looks after me.

My hubby had an attack in the lower left ventricle. He was a successful candidate for Entresto. 2x a day. He has COPD as well. Sprivia has no steroids so is suited for him to take. I know I am really late for a reply, but I wanted to let you know as of 2019 there are options for you. Thank goodness he is now on medicare for the cost would be overwhelming for us otherwise.

Oh yes. No hospitals and no issues since he has been on these meds for 2 years now. He even mows the garden. At one time before the miracle drug (Entresto) he could barely lift himself out of a chair. I would say he was a 1000% improved.
