Hi, was just discussing this the other day. I get this happening, yes extremely painful. The only way I have found is to put your arms out straight, above your head to stretch your rib cage apart with slow controlled breathing or hang by your hands from a door frame taking your weight. The muscle should pop back in, it will be sore for a day or so afterwards.

Try to avoid coughing too hard and use huffing instead to move the mucus first.

I get the same thing, it usually happens if I bend down when I'm in a sitting position - such as when I'm putting my shoes on. As 2greys says, stretching helps to pop the muscle back in place, I do gentle side bends while massaging the area before doing a full stretch.

I found simple stretch exercises as in Tai Chi helped me enormously not only for muscle pain but all areas including my breathing and joints.
