Here's a spreadsheet that I keep which details which users have been banned, suspended or warned for bad behaviour. It also keeps track of people who have created multiple accounts.

Because generating an HTML page from the spreadsheet each time is a pain, I've decided it's just easier to host the spreadsheet instead! The most recent version will always be here, and I'll add a post to this thread whenever it gets updated:
