Hi all,

Can you help us by providing us with information on how we're doing & what you'd like from us (APP) in the future? We also need to show our funders how we've helped people over the past year, so if we’ve helped you or your family in any way, we’d love to hear about it!

If you’ve had any contact with APP or used our services or resources in the past year we'd be grateful if you could tell us about it. This includes the fabulous community here, the APP website, Facebook/Twitter or if you’ve seen us at events etc.

It shouldn't take too long & it'd be really great if you could complete it within the next week if you can - later is also good though. All surveys are confidential.
