I found a paper titled "The Five-Box Thoracoscopic Maze Procedure" by John Sirak et al in the Annals of Thoracic Surgery dated Sept 2010, which is free to download. Never heard of that variation of the Maze myself but that paper might get you started. Figure 4 shows the five box pattern.

In terms of finding a centre that does it, I think that you'd need to start talking simply about the Maze procedure and bring up the 5-box method then.
Perhaps someone with more experience of the Maze can post.

Are you familiar with Dr Wolf in Houston, the inventor of the Wolf minimaze?
And he has a colleague in Tokyo, Dr Ohtsuka, doing a similar procedure.
