Hi there, I’m having a BCC removed from my arm, I have PAF, should I ask for anaesthetic injection that has no Adrenalin.

Thanks.

I've had lots of little BCC's removed. Have never asked for an adrenaline free injection, but did mention it a few weeks ago when I had one removed. The nursing team were quite taken aback when I said adrenaline can have an effect on our heart rate. I didn't ask whether they used it that time or not. I was too afraid that an adrenaline free one would not be strong enough to kill the pain. I certainly didn't have worrying AF afterwards. In fact I don't recall having AF after having any of mine removed.

About two years ago after having a squamous skin cancer removed from my thigh, I was sat in my friends car feeling really sorry for myself as she drove me home. On the way I looked out of the window and saw a young man sat on a bench, he had lost both his legs from the knee down and wore what I would call running blades. My goodness that soon cured my self pity and I felt total ashamed of myself.

Hope all goes well for you and you heal quickly.

Jean

Hi Jean, thank you for your reply, I had it done this afternoon, I did mention it and just like you they looked amazed, asked where I’d heard it! They said loads of people are on Apixaban and they’d never used a different injection. I did bleed more than previously and it’s bruised, but heart rate okay so far okay, so fingers crossed.

Yes you’re right, let’s count our blessings 💕

My dentist told me when I asked for an adrenaline free jab that they hardly worked at all and she would do the procedure without a jab! It went fine but I think she has me down now as a no jab person and my last visit to have a crumbling tooth fixed was very painful so I think the next time I go I'll go back to the adrenaline jabs. I'd rather risk an afib event than put up with the pain. They never put me into afib in the past anyway.

Yes, my dentist told me the same - that the adrenaline jabs weren't as strong as the ones with it. My heart often takes off after I leave my dentists even I've just been for a check up. All I can think is it must be the position of the chair or keeping my mouth open while he does a check up.

Or possibly because the entire dentist visit experience is horrid!? I dislike my dentist as she constantly nags me about my teeth grinding . Also she has got less dextrous over the years and the experience has become more painful. She's probably coming up to retirement. At least she's given up smoking, She used to stink of Gauloises.

It's not that the adrenaline jab isn't as strong. I always ask for it at the dentist's. It just doesn't last as long and may need to be topped up.

The absence of adrenaline just essentially shortens the duration of numbness, but you still get your anesthesia ,it just will not last 2-3 hours but only 45-60 minutes which is usually plenty for dental work or simple procedures.

Good to know . Thanks

The early NHS trained us well to endure dental treatment without anaesthetic. I remember having a filling done as a child, I received a lecture about eating too many sweets (we were still on rationing !) and just had to grip the arms of the chair while he drilled with the slow speed drill of that era !

That would probably count as child abuse now!

It is definitely worth talking about this and reminding the surgeon if your health issues and medication. It's often advisable for people at risk of aFib and with certain medications to avoid local anaesthetic with a adrenaline, myself included.

Thank you, re my reply above, I did mention it but they weren’t bothered.

You can request to have the local anaesthetic without adrenaline and check before it's administered. I have to do that , it's worth taking the precaution just in case Adrenaline is a trigger for you.

I had a bcc removed from under my eye about 18 months ago . I had the standard injection and no more bruising than you would expect from anyone . I take Edoxaban . The anaesthetist said my heart rate was 58 throughout . I think they use a lot more anaesthetic if doing say a dental extraction . Problem is, the Adrenalin in local is to cut down the bleeding , obviously helpful if on anti coagulants. Another incentive to look after your teeth 😊

I have had a number of BCC’s removed in recent years including a large one on my scalp. They gave me deep sedation and I had no problems

I always get adrenaline free anaesthetic when I have a filling at the Dentist and it certainly makes a difference as I used to experience palpitations beforehand.

Never noticed a problem in a Hospital setting when I’ve had a local anaesthetic for stitches etc

Did the adrenaline free anaesthetic work? Thanks.

I had one removed from my forehead last year and have PAF. Knew nothing about this and I was fine. Think he just gave me a series of small local anaesthetics . Felt nothing and all over quickly. All the best.

Thanks folks, I do need a dental extraction soon so will ask the dentist. It’s a bit of a catch 22 situation really because the Adrenalin helps stop bleeding and it did take a long time for the bleeding to stop yesterday, I wouldn’t want to exacerbate that especially with an extraction. I’ll perhaps just trust my dentists decision.

For many years before my AF started, I had noticed the adrenaline injections immediately sent my heart racing. Personally I would ask for non adrenaline injections every time.

Had lots of dental work and skin cancer work with the Adrenalin locals and never has it set me off. Now, a dental cleaning with all the scraping and without any pain relief … that’s a different story.
