So you have heritable dilated cardiomyopathy with AF, a pacemaker, take warfarin , bisoprolol and diuretics. And you support the Millers who have never made it to the top tier unlike my team Charlton.
Is that about right?
Your ankle is swollen and red. Is it warm and tender or painful? Have you got a fever? Can you stand on it?
You asked what that could be.
Deep vein thrombosis not so likely if your hopefully recent INR was OK.
Superficial thrombophlebitis, septic arthritis, gout maybe.

I agree, it seems to be an acute, unilateral, local inflammatory condition not related to cardiac disease. Being on Warfarin will rule out a thrombotic event. Most likely a local flebitis or skin infection. Gout or septic arthritis would be producing excruciating pain and she would have gone to the emergency room of a hospital already.
