I would think they will put a pillow under his knees to decrease pressure on his back. Maybe he can request it if they don't. My sciatica and asthma demand a pillow under my head and legs when lying down. Good luck to him with his procedure.
Don't worry it will be fine. They will spend plenty of time making sure he is comfy at the start. They can always administer sedation as well if needed.
They are very unlikely to do it just with local anaesthetic. He will almost certainly be given sedation which in normal circumstances causes amnesia and therefore he will almost certainly remember very little.
Regarding the cough hopefully it will have eased by Monday.

I was sedated and although awake, I was in lala land. Time flew by and I felt totally relaxed. At one point when they started ablating, it hurt and I groaned, then they knocked me out immediately, next thing woke up in bed all done.
And it worked perfectly, it was marvellous.

One of my worries was wondering what happened if I wanted to go to the toilet during the procedure! I needn't have worried, as the sedation really worked, and before I knew it, it was all over.
