Hello Everyone,

I was previously on the PSMAdditions trial since January 2023 with 6th and final infusion in August 2023. My PSA was undetectable until several months ago. I had a PET scan in October that showed new lesion in my acetabulum. So I had external radiation to the area but it has spread so fast. Now, PET scan from this past week show the following in the bones:

Bones: Significantly increased in size and tracer uptake of right posterior acetabulum metastasis with interval extension into the right iliac wing and right ischium with max SUV of 26.7, previously significantly smaller osseous lesion with max SUV of 24.5

Multiple new tracer avid lesions compatible with multifocal osseous metastasis involving the axial and appendicular skeleton. These includes:

* Skull, including the left parietal bone, right sphenoid, right craniocervical junction,

* Spine including: C1, C2 (max SUV 14.9 at dens), C4 lateral mass, C5, T2, T7 T9 (max SUV 15.1), L1, L3, L5 and sacrum

* Bony pelvis: Aside from the large right acetabular/iliac lesion discussed above, there are scattered new tracer avid lesions in the right iliac bone, left iliac bone, left superior pubic ramus, right pubis, right inferior pubic ramus/ischium with max SUV of 15.0

* Multiple bilateral ribs, including the left 11th rib with max SUV of 9.6 and right posterior second rib with max SUV of 8.9.

* bilateral scapulae, right sternum

* Left femoral neck, bilateral mid femoral shaft,

My MO wants to put me on Xgeva and Docetaxel, but I found this clinical trial ID NCT06402331 that is close to home. I have concerns in regard to potential side affects. As of now it appears that my MET’s are in the bones only. But it is spreading very fast.

Any suggestions or experience about either Docetaxel or Actinium 225 for treating metastatic prostate cancer in the bones would be most appropriated.

I had 6 cycles of Docatexal November 2023 to March 2024 as palliative stage 4 treatment and had minimal side effects that never prevented me having a fairly normal life .

A subsequent series of scans showed my cancer had shrunk right back and enough to enjoy a holiday abroad in Spain using insurance advised by Macmillan Cancer Trust.

12 months later my cancer has returned to the point I now need further chemotherapy and have currently just had cycle 2 of Cabazitaxel and so far coping well.

I have realised for some time now that I am on end of life treatment but giving it all I can to last as long as I can in a healthy normal life condition and doing pretty well for now.

Good luck whatever route you take.

Thank you for the information. I sincerely hope Cabazitaxel works well for you. May I ask if your Mets are in bone alone or also in organs or other soft tissue?
