Has anyone tried taking erleada after the failures of xandi(enzalutamide) and zytiga(abiraterone)? If so, how did it go?

benn there. Watch your weight, blood lipids especially fasting triglycerides. Mine went up fast and decided to stop it because of increased cvs risk. I had history of stroke.

Put myself on zero alcohol and avoided all sugar and seeet food for a month and all returned to normal.

thanks a lot Allen but we have already done docetaxel and it did not work at all. the sad truth is that there are several new generation hormonal drugs (enza, abi, apalutamide and darolutamide) but when one of these fails, you are in big trouble because the others hardly work. I read about Magnus a member of this forum who did very well with several molecules (and I am very happy for him) but unfortunately it is a situation that occurs very rarely.

BAT is being used to “resensitise” the tumour to anti cancer therapy . Basically it uses high dose Testosterone to achieve this. Google BAT trial for CaP

I started my treatment in January 2024 with Abiretarone/Predisone combined with Orgovyx. The Abi wreaked havoc on my liver panel scores, especially AST and ALT....We tried lowering Abi, taking a vacation and restarting Abi. The liver counts kept going up into the danger levels, so we dropped the Abi and tapered off the Prednisone. I have now been on Erleada (Apalutamide) for almost 6 months...So far so good, fingers crossed. I wanted to switch to Nubequa, but M.O. was concerned the same thing (elevated liver counts) would occur. Best of Luck !!

thanks good luck to you too! in your case abi was suspended for side effects, not because it failed, so you will surely have a good run with apalutamide. I wish you many years with it

Apalutamide will work for several months after Zytiga fails. You may want to get a Pluvicto therapy or Cabazitaxel. aacrjournals.org/clincancer...

I began treatment for Stage 4 with Erleada and Lupron and I'm entering my 5th year. The usual listed side effects are with me but it has maintained T 0.5 and PSA 0.02 since starting.

Apalutamide, sold under the brand name Erleada among others, is a nonsteroidal antiandrogen (NSAA) medication used for the treatment of prostate cancer.[2][8][9] It is an androgen receptor inhibitor.[2] It is taken by mouth.[2][10]

Side effects of apalutamide when added to castration include fatigue, nausea, abdominal pain, diarrhea, high blood pressure, rash, falls, bone fractures, and an underactive thyroid.[2][11][12][10][13] Rarely, it can cause seizures.[2][10] The medication has a high potential for drug interactions.[2][10] Apalutamide is an antiandrogen, and acts as an antagonist of the androgen receptor, the biological target of androgens like testosterone and dihydrotestosterone.[2][10][14] In doing so, it prevents the effects of these hormones in the prostate gland and elsewhere in the body.[2][10][14]

Apalutamide was first described in 2007, and was approved for the treatment of prostate cancer in February 2018.[8][9][10][15] It is the first medication to be approved specifically for the treatment of non-metastatic castration-resistant prostate cancer.[2][10][9]

I have been on Erleada and Eligard since my diagnosis of gleason 9, stage 4 metastatic PC ( lumbar spine and lymph npdes).In other words I am not yet castration resistant. My PSA fell from 195 to 0.01 and it is holding steady. I did have a severe spike in my triglycerides but resolved that with high doses of Omega 3 and 1000 mg of Niacin.

As a side note, I had a catheter put in at the time of my diagnosis. Due to severe penis pain following radiation therapy in February, I decided to finally get surgery in an attempt to eliminate the need to continue with the catheter. Got the surgery one month ago (HOLEP) and I am currently pain free and pissing like a champ. No incontinence, no urgency and no frequency.

Thank you so much for the HOLEP suggestion. That’s what I need or something like it. Urination issues negatively affect me 24/7. There are other procedures I need to check out, PAE, PVP and Rezum steam treatment. The last one seems interesting.
