I've just updated my husband's profile. He completed 6 of 10 Cabazitaxel treatments in July of 2024. Scan in July and scan in Sept showed no changes. He had a PSA taken yesterday and his PSA has more than tripled to 42.9. He has an appointment with the oncologist on monday. At the last meeting with him, he had said probably no more traditional treatment only trials would be available. We had mentioned a PSMA pet scan previously but he has never had one. Would this be beneficial at this point? He does not have the BRCA gene. Would some other genome or genetic testing be beneficial? He's feeling well other than some tiredness. This PSA result is quite upsetting and just trying to find the next plan of action for him. Many thanks to all here.

I am NOT doctor I am NOT Medical Advisor. Here is my personal opinion. It is not medical advise.
- Have you track down his testosterone many times testosterone level is up that cause PSA go up. I do not see any informations about testosterone in profile
- He should do PSMA PET SCAN Immediate basis and try to find out how many mets.
- Many times SCAN show stability or improvement and PSA is going up in that case, Continue do scan every 4 week (CT SCAN + BONE SCAN )
- Talk to doctor about PLUVICTO or Xofigo® (radium Ra 223 dichloride). Xofigo® (radium Ra 223 dichloride) is good target therapy for bone pain
- Doctor will not prescribe for PARP in absent of genome
- To answer about clinical trials, - It is good options and also depend which clinical trial. Please post the questions about clinical trials that offer by doctor and find the patient that already enroll in the clinical trials. I think it is direct inputs from patient help a lot.
- Also setup appointment with Radiations Oncologist and other Oncologist for second opinion, here many user have 3 + oncologist
Thank you for your input. His oncologist has not given him his testosterone levels, just has said they are OK. Will get that and push for the PSMA Pet scan. I don't believe Pulivcto is available in the UK through the NHS, yet. Again, thank you. Will have more questions for Dr. monday and get some more answers.
There are many patient going India and Turkey for the treatment like Pluvicto and actinium 225. There is one user posted here. In India there are doctor who combine pluvicto + actinium 225 for infusion. I am telling you because your name suggest me that you are Indian

Is Xofigo NHS-approved?
There's no value in a PSMA PET scan for him because Pluvicto is not NHS-approved, unless you want to go to Germany for it.
DO you mean that he has a missing BRCA gene? PARP Inhibitors can help a lot if BRACA is missing.
Foundation One NGS of my Dad mentioned missing BRACA2 gene after which the Oncologist added Olaparib (PARP Inhibitor) to his medicines. Dad has been feeling much better since then. His PSA levels have improved too.
My husband did Foundation One testing at his own expense. No actionable mutations were found but the report informed him that wasn’t resistant anymore to Zytiga. We are one month in with it and first PSA reveals that it’s working
Thank you, will see what we can do with the Foundation One testing. Since he's already tried Xtandi/Enzalutamide the Dr. had mentioned before that he couldn't try Zytiga/abiraterone. He had the choice of one or the other. Will see what he might say now. Thank you.
Is he on ADT drugs (lupron or orgovyx) as well as one of the receptor inhibitors {Xtandi or Darolutamide}?
I am a patient who is making this clinical trial clinicaltrials.gov/study/NC.... I'm afraid there is no.locations for UK.I only have cabazitaxel option but they offered me ghis trial thats is working quite fine to me.
