After a year the second scan produced the following impression from the radiologist, "1. Marked improvement in radiotracer avid osteoblastic metastasis and marked improvement in radiotracer avid nodal metastasis. Radiotracer avid disease in the prostate has also markedly improved." Bear in mind that I was diagnosed as stage 4, Gleason 9, and I'm now 87 years old. I've been on Orgovyx and Zytiga. Furthermore, my PSA has slowly climbed from a low of 0.69 to 1.89. It's great that many of the small cancers disappeared, and the larger ones have shrunk and I do not have any bone pain. But, my question is essentially now what? After the first scan the radiologist said the cancers lite me up like a proverbial Xmas tree. Terry

87 is pretty impressive.
Stick with Zytiga until radiographic progression. You can switch from prednisone to dexamethasone.
