Had a PSMA scan that showed 2 Mets, one to lymph node, the other to a rib. Well there is no cure for me now. The met were radiated about a month ago but my side where rib is still sore. I assume the cancer is growing. They decided to not use ADT. I know that I am at the end. So getting my affairs in order. I read that a cure is possible with olio metastatic but that’s fiction and wishful thinking. I am 78 so three mo years would be great. I had surgery in 2014, Gleason 7. Had salvage radiation two years later, radiation to pelvic lymph nodes two years later and now radiation of bone met and lymph node. Radiation screwed up my bladder so I have terrible incontinence. "They decided to not use ADT". If this is the case they do not think you are in a bad situation. If you start with ADT plus Apalutamide now you will live for several years.
You still have many treatment options, ADT, second generation ADT’s, Xofigo, Pluvicito, etc. You should fill out your bio completely. I’m sure many others have been in your position and can share what’s worked.
Why no ADT + abiraterone or enzalutamide?

Was ADT offered back when PSA recurred high enough for radiation to prostate and then to nodes? It would usually be used now after radiation of bone and node mets. What is the PSA now? A doublet therapy of ADT and an ARI would add years before the end, unless there is a good reason not to use it.
I’ve had 2 mets on different ribs radiated, both times the rib was sore a few weeks afterwards, and the radiation was successful, shown on subsequent scans.

Reading through your previous posts- looks like ADT worked for you before you stopped taking it. What reason are they giving for not restarting ADT? Many men on here have mets and have lived years- my husband included.
It's not over for you. You aren't timestamped to die. I understand your point of view. I have been struggling with PCa for as long as you. A time comes when hope for a prostate cancer cure isn't the light at the end of the tunnel. Think of that tunnel as not being a dark place. It is a place you can fill with light, not with the hope for a cure, but with the little joys of living within the moment.

At the end of what? A lot of people on this list, myself included, have metastases and we haven't given up. Why should you?
Howard, I am at 24 yrs from diagnosis of Gleason 7 PCA, and have had many years of multiple treatments. Have had 12 bone lesions radiated and 3 treated with cryotherapy. 2,5 years ago, had PSMA and Glucose PET scans showing several lesions, with PSA of 12. Started LU177 2.5 years ago. responded to PSA 0, although on the fifth cycle I received an accidental triple dose which resulted in severe bone marrow dysfunction and renal failure. Anemia resolved after 6 months. Renal failure resulted in need for dialysis, which persists.
However PSA remains 0 at 2 years duration, PET scan shows no active lesions, showing only evidence of scattered cold lesions. I have been on no therapy at all for 2 years. My oncologist, Chief of Uro oncology at Houston Methodist and former full professor of Oncology at MD Anderson says I am cured, and specifically not just in remission. Time will tell, but so far no evidence of disease,
Therefore, do not give up. Treatments are continually evolving.
