Finished my second treatment last week.

My PSA went from 10 to22 after the first round. Today after the second round it dropped to 15.

Anyone else have a jump after the first treatment and then continued to decline for the remaining treatments.

I have failed chemo and xtanti and have read that combining xtanti to Pluvicto could increase the effectiveness but my medical oncologist is reluctant to add it.

any comments appreciated.

PSA is not a good indicator of success or failure until after several treatments and some additional time. There can be PSA flares caused by dying PC cells that does not indicate failure. You have to be patient. Same is true for my EBRT. I was told to not bother getting a PSA test until a month after the last treatment as it will not be diagnostic earlier than that.

I had the same results, a flare after first treatment and then a small decline. Pluvicto was stopped due to blood test results. Then PSA continued to rise again but a little more slowly than before Pluvicto

I had 3 rounds of Pluvicto and then had to stop. He had me discontinue the Lupron and Zytiga I was doing during the treatment. That was a big mistake, The non PSMA cancer took off like a rabbit in heat. I did not do Chemo first.

I recently did a new treatment for my liver Mets, called Histotripsy. I am now back on Lupron, but will soon start a recently approved drug, by pilled called, Lynparza. Going to meet with Oncologist In July and go over the side effects, etc...

As others have said, early PSA evaluation has not been the best way to approach continuing (or not) a treatment.
