Updated: Father with aggressive cancer psa 2000, Hemoglobin 7

Not sure what’s happening. It’s not vomit. He keeps spitting blood with Saliva without his knowledge, no pain. Not a dental issue either.

It looks like clot. No clue. Please help with your advice.

may be low blood platelets? I had similar thing from nose

Yes, his platelets are low 105000. Also hemoglobin is 7. Does blood transfusion help?

Bleeding from low platelets doesn’t generally start until you get below 20,000 and serious bleeding below 10,000. 100,000 is enough. He needs and exam of his mouth, throat and nasal cavity. You didn’t mention cough which makes lungs less likely but it would be another possible source.

Once infusion will help that will help him with low platelets and hemoglobin.

Also check iron and other blood work level. I hope dr can tell what are other treatment help him to bring his hemoglobin up as well.

Yes that is pretty low and suggests bleeding somewhere usually GI tract. Chemo could be contributing as well .

I don’t think so. He is done with majority of the treatments. His body is not helping withstand any treatment. Managing his condition and taking care of his clinical wellbeing seems better option at this point, than addressing the disease itself. Thoughts?

It is my personal opinion, I do not agree. I seen worst case than this. Also Quality of life going to so worst.

From all past communications, I know he is in good health and no side effect except this one. I will strongly suggest for clinical trials of any ADC.

I think ADC is good options.. ADC has min side effects.

I was at hemoglobin 7 and psa 200+. I needed a transfusion and the treatment of Lupron and Nubeqa. That brought be to good health in four months and lasted for five until it stopped working. I was too weak for chemo. Your father may be in the same boat. It’s something you can fight and win! I am now on a PARP inhibitor.

Lupron is a form of ADT and is usually still continued even after ADT fails.

The PARP inhibitor is one of many treatments that can be used on some patients after ADT and Chemo failure. PARP inhibitors work better on patients that have some types of genetics such as a BRCA2 mutation.

Done with all treatments. His last treatment was Cabazitaxel, 3rd and final cycle finished in Feb 2024. He is just being taken care at home.

UPDATE: Done with blood transfusion. He stopped spitting blood. He is still weak but steady and OK.
