PALB2 Stage 4 A prostate cancer treatment plan.

My Partner has stage 4A prostate cancer spread to the nodules and has the PALB2 gene. How effective is internal and external radiation and ADT as the first line of treatment? Why can't the drs start with PARP inhibitors and chemo which is the standard of care as a second line treatment? Is it because the FDA has approved the latter only if castration resistant?
