Gleason 7 4-3..IMRT 20 txs finished 2/28 No after effects went fine except for 4 months adt (orgovyxx) ..still little recovery (T-98) 1 year after ending adt 5/23 recovery only to 98 in one year , baseline before adt 286 with everything working fine.
Stopped 5/23 Orgovyxx Still Hypogonadal ( T-98) 1 year later.
Thinking TRT- worried if I start have to stop due to TRT Side effects , A-fib, acne, cardiovascular , sleep apnea, loss of bladder control, burning, pins & needles, etc.)
Can you try TRT out for a month and see if side effects and if bad SE's can you go back to "natural" recovery or have your testicles shut down production of T forever and ARE YOU STUCK THEN WITH FOREVER HYPOGONADAL SYMPTOMS (no libido???,fatigue, weight gain ,etc.)
Can you titrate down or up with gel to reduce side effects?
Mohit Khera out of Baylor recommends using HCG instead , still gives you the LIBIDO back you want with TRT but no Side effects ? (I read Pub Med and there are plenty of Side Effects with HCG? Has anyone taken HCG or been using HCG and know of Side Effects?
Khera also states that if you lose 20 lbs, your T will increase 100 ng's of T? ( that would be close to 200 for me and ok?)
5. Anyone know of Testosterone or TRT (sex mens health Doc) anywhere in the country who knows T or TRT really well..Please advise... Thanking you in advance for all responses!!!

"your bound testosterone may be excessively bonded to SHBG, which can decrease the amount of free testosterone needed to perform many of the functions responsible for developing and maintaining your physical characteristics. A decline in your free testosterone production can lead to some of the symptoms you’re experiencing, such as reduced muscle mass, lowered sex drive, mood changes, and other symptoms that are the most common signs of testosterone deficiency. "
."..In addition, you may also need to ask them to check for other biomarkers, such as SHBG, albumin, estrogen, and estradiol, to determine if other hormones have been affected and may be interfering with your testosterone production."
Gonadotropin-releasing hormone (GnRH) is synthetized mainly in the hypothalamic pre-optic area and is the ultimate regulator of gonadotropin release from pituitary
"ORGOVYX TESTOSTERONE RECOVERY 90 DAYS AFTER DISCONTINUATION1,2,4
Testosterone recovery was evaluated in a substudy of 184 patients who completed 48 weeks of treatment2*
90 days after treatment discontinuation, 55% of 137 men treated with ORGOVYX had their testosterone return to above the lower limit of the normal range (>280 ng/dL) or baseline values1,4*
3% of 47 men treated with leuprolide had their testosterone return to above the lower limit of the normal range (>280 ng/dL) or baseline values 90 days after discontinuation4
This endpoint was analyzed for exploratory purposes without formal testing. The data from the leuprolide arm were not included in the US Prescribing Information for ORGOVYX"
"AMA Oncology, Dr. Atish Choudhury, a genitourinary medical oncologist and the chair of the Gelb Center for Translational Research at Dana-Farber Cancer Institute in Boston, noted that patients experienced faster testosterone recovery when treated with the oral drug."...
"The injectable treatments, Choudhury explained, work by initially overloading the body’s testosterone production machinery, leading to an overdrive of the system that eventually causes it to shut itself off. Orgovyx, conversely, directly blocks the production of testosterone.
“So, rather than overdriving the system and then locking it up, [Orgovyx] actually locks it directly so that the testosterone never has a surge at the beginning so that testosterone becomes lower much faster and stays lower during the course of treatment — what we call the sustained castration rate in these particular trials,” he said. “But, also interestingly, after you stop, it wears off more quickly and then the testosterone recovers more quickly.”
For example, researchers noted, in the Orgovyx and Firmagon patient arms 52% of patients (34 patients) and 16% (six patients) had testosterone levels return to baseline within 12 weeks of discontinuing treatment.
"...Furthermore, researchers stated that a previous study showed that among patients receiving long-term androgen deprivation therapy with an injectable drug at a median duration of 24 months, 47% of patients did not recover to normal testosterone levels within two years after stopping.“In contrast,” they wrote, “even after 48 weeks of [Orgovyx] treatment, most men recovered to normal testosterone levels within 90 days after discontinuing ADT.”
"According to the results of a clinical trial published in the New England Journal of Medicine comparing standard Lupron (leuprolide) therapy to Orgovyx in patients with advanced prostate cancer, Orgovyx was associated with sustained testosterone level suppression and a lower risk for major cardiovascular adverse events than Lupron. Are you saying taking avodart and bicalutamide increased your T by 30-50% and 50-100%...were you hypogonadal prior..could you please give me some more info jeffreyslaw@aol.com..thanks
Yes, as to the increase, no, as to the hypogonadal. It is well known that these two drugs increase the endogenous untreated tT. Not sure if this still applies after ADT castration though. They will also help you against PCa, but your sex drive will not improve at all, if this is the reason you are after.
Sex Drive besides other things is my main reason..have 34 y.o . wife..I think I took avodart years ago for bph and it in fact reduced my sex drive,,,so you can increase the T numbers without increasing sex drive???
