had bone marrow biospsy and the mCSPC has spread bone marrow. They are recommending thaat I stay in hospital replace Abiraterone with Xtnandi. Thoughts?
2nd question my red blood 2,38 and platelets 23. have plummeted, they want me in hospital. How can I manage the red blood and platelets outside the hospital.
Hospital is close. There has tv ways to prevent the numbers fom plummet ing.

I’m not aware of any visiting nurse that gives transfusions at home in the US. One has to be monitored for any reaction. Can’t treat those reactions at home.
Blood transfusions are going to be needed, it takes about 2hr/unit of blood +/- waiting time. So not a lot of time you have to be in the hospital, but best to get the transfusion in the hospital for appropriate monitoring. This will help RBCs and Hgb, but not Platelets. They will come up with recovery of the bone marrow/reduction in toxicity. Platelet transfusion effect if very short lasting (less than a week), so not used unless you need to go for a procedure or treatment that needs temporary boost.
I also have disease in my bone marrow and being going thru transfusions. It's not easy to recover from it. Good luck.

Context: the care giver is giving me the option of staying in hospital and having daily transfusions or entering hospice and ending the transfusions. How long can I survive with low red and platelet counts?
2nd question, any reason to believe that Xtandi can arrest the bone morrow cancer growth?
Really sorry to hear that. I have been fighting bone marrow cancer issue for good part of this year and I consult with 5-6 oncos around the country. No one has mentioned that Xtandi can help with it, as I am mCRPC. Best would be to talk to your onco on the topic. Good luck!
