dx 2017 psa 489. Did Chaarted trials 2019 started Zytiga. 2021 oriechtomy/discontinuation of Lupron. 2023 Zytiga began to fail. Psa rising. Did Radoum 223. Been a few months and been with no treatment in 6 months psa went from 3 to 7. Last week PSMA showed most stable disease from radoum but two stubborn spots that seem to be a problem and Mo wants to get it under control and is suggesting a Re challenge of Docetaxel since he did so well on it when he was hormone sensitive .
What other treatments are an option outside od Pluvicto- I’m concerned don’t want to burn through treatments and want to think outside the box and see what other options may be available for him.

I hope the rechallenge is successful! For my father it didn’t go so well.. I felt like his oncologist let him down too. I wish she would have swapped him over to cabazitaxel sooner. I would recommend having your plan b and c established as things can move quickly as we sadly found out. Not saying this to frighten you but I wish we had been more prepared and Pluvicto was more readily available when it became evident that Docetaxel wasn’t working the second time around. Cancer doesn’t wait. Crossing my fingers it goes well for you!!! Stay strong!!
If there are just 2 spots that aren't responding to systemic treatment, spot treatment with SBRT might keep it under control for a little while longer, perhaps with added enzalutamide. It won't be for a long time, however. Maybe worth a discussion.
