Ok so up front: Gleason 10. Matters? Dunno..castrate resistant now as aberaterone failed.

Been battling this since 2020. I was PSA .12 this time last year, started going up at Nov 3-month just, and up since, which was apparent failure point for aberaterone. I had cancer jump into hip, hip muscle, lots of pain that immobilized me. Radiation in Feb. Given pain meds. They finally found fractured hip, so surgery in June, more radiation. PSA kept going. Started chemo 2 weeks ago. This spring and summer I've slid way back, lots of muscular atrophy and weight loss. I'm getting ahead of the pain, at least for now. I'd like to try and get as mobile as possible, and try to recover some form of muscular work, minimally for some endurance. I have medical care thats good at chemo and radiation but not so good about nutrition and whole person health. So gents I'm asking for any links, advice, and resources you have for physical rehab, being wary of course of the Gleason aggressiveness of my initial diagnosis.

Building muscle and losing weight are hard to accomplish as muscle growth is best under high caloric situations but it can be done. I would start with a workout program assisted by a trainer familiar with your condition, if you can't find one then explain your issues to them and they should be able to help. They can also help you with the weight loss with a good nutrition program so as not to lose muscle while losing weight. A friend of mine uses an online trainer that gives her a workout for every day as well as weekly meal plans

I've been learning about blood flow restriction training for muscle development, and it's possible it could help you a lot. If it sounds bogus or snake oily, keep reading about it because it isn't. When used properly, it is a safe and science-based tool of physical rehab. I don't have any personal experience with it yet; I've ordered some cuffs and am eager to get started with it.

Whatever you can do safely, do as much as possible. Get a trainer if you can. The hip obviously is constraining but there is much that can be done working around it. Exercise will stimulate your appetite and change everything for the better, but you already know that.

You might consider a "chest expander". The "Big A" sells a UEASY brand with five tubes which can be adjusted easily using one to five tubes. There are different levels of max resistance available. There are many exercises that can be done and almost anywhere, many while sitting.

You might check with your healthcare providers and see if they can order some physical therapy for you. Working with a good PT could help.

High gleason here. Seems no booze, mainly whole food plant based eating, small servings of chicken and steak (few serves a month) breakfasts of organic oats, organic coconut milk, blueberries, kiwi fruit, apples, lunches of lentils, kale, beans, corn has helped contribute to an extra 7 years of good health, travel and great memories. Dr Michael Greger and James Duigan ( a lifestyle and exercise trainer) both have great info on their websites.

Three thoughts for you DD50: (One and Two are solid. Three is a risky wild-card)

One, on exercise- Safety is paramount for you. You cannot afford an exercise related injury. I suggest using well designed machines under the supervision of a trainer. Once a week is enough if done right. The two best machine systems are the MedX and the Naughtilus. See if they are anywhere near you in a gym or Physical Therapy office. Read "Body by Science" for the what, why and hows of "The Big 5" exercise program. Available on Kindle. The details are important! It works!!!

Two, on Nutrition- Get enough protein regardless of your chosen diet. Supplement high quality protein such as whey shakes (sugar free) 25-30 grams protein once or even twice a day.

Three, More controversial and not SOC with mCPC: SARMS such as Rad140. Androgen signaling is very important to muscle and strength as well as to brain and nerve function. You cannot take testosterone (such as BAT programs) because of your painful bone mets. But SARMS (Selective Androgen Receptor Modulators) may allow for substantial muscle rebuilding and better response to exercise. Rad140 is one that does not activate the canonical (normal) androgen signaling that testosterone does. So it may be possible for you to use it without accelerating your prostate cancer. You would have to monitor PSA etc to verify that it is not making things worse. It is a risk and must be carefully considered and weighed vs potential benefit. Rad140 can be bought at SARM sites on the internet where you can also find other information (dosing, side effects, etc. I can send you an article reviewing SARMS if you would like too consider more deeply.
