What’s Next After Lupron and Aberaterone?

Dx almost 11 years ago with about 4-5 tumors in bones- the largest one in the left acetabulum. Had the prostate removed shortly after Dx along with a bunch of lymph nodes. Basically, I have been on Lupron and Aberaterone for most of my 11 years except for 3 “vacations” during the first 5 years. PSA has basically remained at below .05, except for the vacations and until about 18 months ago when it became measurable and started to rise. PMSA scan showed the largest tumor to be the culprit so we radiated it last June and PSA became immeasurable once again until 1 month ago when it was at .05 and then .06 a few days ago. MO wants to keep me on same treatment as scans will not show any changes with PSA this low. Is that the right move? And, what next step is suggested when PSA reaches .2 and scans show tumors growing?

After Lupron and Zytiga failed, I enrolled in the LuPSMA177 trial at Dana Farber. Initial positive results but pSA continued to tend upward. I have moved onto a BAT protocol. Responding very nicely.

Great news! how long have you been on bat? Are you doing SOC BAT or Russ’s modified BAT?

So you went castrate resistant before BAT? What was your PSA prior to BAT?

I’ll take a look at your bio. Just read it and realized I’ve read it before. Your doing great considering your past drug failures.

I would love your protocol!

PSA prior to going on BAT above 4.0. Started BAT the day after meeting with Denmeade, Jan. 14, 2023.

I am using Propionate testosterone. Denmeade uses Cypionate, longer decay rate. Propionate has a short half life which many believe contributes to shocking the cancer more, along with very low T on the down side. . There is one compounder state side. I happen to go overseas for mine. The needles and stuff are easy to obtain. The actual program was designed by a fellow prostate cancer pioneer. I would be more than happy to share the program, material, and my results to date. I share my results with 4 other doctors who are following me.

I may be confused but shouldn't it be 2.0 and not 0.2 (but what the hell do I know?).... Hopefully your 11 years will give you at least 22 more.....

Thx. I think you are correct that the number needed for a scan is 2.0

PSMA pet scan could under certain circumstances show something even at PSA 0.2 but at PSA 2 it is 90%. If I were you I would like to have a PSMA pet scan even at only 0.2 but it is up to you and how fast is the PSA doubling time and your symptoms. If you have pain than the scans would show something even at PSA 0.2.

FYI I am getting new 3 month labs in about 10 days and will see if the PSA continues to rise and then discuss with my MO the next move (PSMA or postpone another 3 months)..
