Did he ever received Xofigo, or have a biopsy to determine if the cancer could respond to Olaparib or similar, keytruda or similar and to cabazitaxel plus platinum compounds?
He could have a liquid biopsy to study circulating tumor DNA and do genomic studies of the cancer. Having multiple bone mets and a high PSA, the liquid biopsy could have an adequate yield.
Yes he has had carboraxil and the tests for DNA markers. Nothing was shown.

ADT should be continued because it's part of palliative care. It will help keep the pain lower by not allowing the cancer to have more rapid growth.

My diagnosis was horrible (Diagnosed 09/2018, PSA 1000+), but for some reason I'm still here for folks that remember when was active here. Been busy with normal life things now, got this monster off my back these days.
Anyway - continue ADT because the cancer population are heterogeneous, so keep them down.
For this reason, my Dana Farber team didn't rest with my initial success from ADT, at the time, we just kept adding (concurrently) treatment modalities.

if he does not have painful metastasis to bones or critical spine, then he could try a BAT regimen. It can restore responsiveness to enzalutamide (and probably others) in many, as well as suppressing the cancer in about half. Increasingly being done by community MOs outside of formal trials as it is easy to administer and monitor.
Have you asked about immunotherapy (Provenge)?
It depends on what your husband wants to go through. Follow doctor's advice as I'm sure he knows best what your husband's condition will or won't respond to. There comes a time when enough is just enough.
